Acne is a chronic and inflammatory skin disorder that affects up to 50 million individuals in the United States and is one of the most common dermatologic conditions in children and adolescents (Lim 2017; Nagler 2016). Acne is characterized by noninflammatory lesions (open and closed comedones) and inflammatory lesions (papules, pustules, and nodules). The pathogenic mechanisms include microbial colonization with Propionibacterium acnes (P acnes), sebum production, inflammatory mechanisms, and follicular hyperkeratinization (Eichenfield 2013).

The increasing incidence of Atopic dermatitis (AD) and poor recognition of the disease in some patient groups is a public health concern. AD symptoms and its daily management cause significant effects on quality of life for both patients and caregivers. AD is associated with an increase in other atopic manifestations as well as with serious comorbidities that can greatly affect overall patient health and wellbeing (Silverberg 2015; Simpson 2012; Yu 2016; Zheng 2011).

Metabolic syndrome and associated comorbidities, such as cardiovascular disease, obesity and type 2 diabetes, are becoming more common in women of reproductive age and beyond. In addition to traditional cardiometabolic risk factors, additional risk factors, including pregnancy-related outcomes, gestational diabetes mellitus, polycystic ovary syndrome, and hormonal replacement therapy, confer important implications on women's cardiometabolic health across the lifespan.

Iron deficiency (ID) affects up to one-third of the world's population and is particularly common in elderly individuals and those with heart failure (HF). Many of those affected are unaware of ID and cannot recognize its symptoms. For patients with HF, ID is an independent predictor of outcomes and a major contributor to exercise intolerance. At least half of all patients with HF have comorbid ID; yet, the condition is under-recognized and untreated in most patients.

The potential impacts of influenza coupled with those of COVID-19 have been termed as a potential “twindemic” coming for the US healthcare system. Although this early concern did not materialize, clinicians should pay attention to the diagnosis and management of influenza in the COVID-19 era and beyond, particularly in adults with multiple chronic comorbidities, including cardiovascular disease, obesity, diabetes, asthma and COPD.

Cutaneous squamous cell carcinoma (cSCC) is the second most common skin cancer after basal cell carcinoma (BCC) (Alam 2018).  The incidence of cSCC in the United States and throughout the world is increasing (Alam 2018).  In the US, each year 200,000-400,000 new cases of cSCC are expected, with disease-related death occurring in more than 3000 people with cSCC (Alam 2018).  Risk factors for cSCC include chronic sun exposure, advanced age, sensitive skin to UV radiation, and immunosuppression (Kabir 2018; Migden 2018).

The advent of SGLT-2 inhibitors and GLP-1 RAs to address cardiorenal risk in patients with and without type 2 diabetes, has opened up new avenues of prevention and treatment. However, individualizing the vast amount of evidence to patient care remains challenging. During this case, you will have a chance to explore treatment decisions to decrease overall cardiorenal risk in a complex patient, accompanied by expert commentary and brief review of evidence with SGLT-2is and GLP-1 RAs across cardiovascular outcomes trials.
